We'll send your forms before you arrive
Your forms cover health history, medications, consent, and HIPAA acknowledgement. When you schedule, our front desk can send them to you to complete at home — or you can fill them out at the clinic when you arrive. Call 503-245-5710 with any questions.
Bring these to your first visit
- Photo ID (driver's license or state ID)
- Insurance card(s) — primary and secondary if applicable
- Workers' comp or MVA claim number, if relevant
- A list of current medications and dosages
- A referral from your provider, if your insurance plan requires one (many do not — Oregon is a direct-access state)
- Any relevant imaging reports (X-ray, MRI) or your physician's notes, if you have them
- Comfortable, loose clothing you can move in — shorts and a tank top are great for knee, shoulder, or movement assessments

Arrive 15 minutes early
Plan to arrive about fifteen minutes before your first appointment so we can get you registered and ready without cutting into your one-on-one time with your therapist.
Confirm insurance the day before
Our front desk verifies coverage ahead of your visit — if you switched plans, call ahead so we can re-run your benefits and confirm whether a referral is needed.
