Creekside Physical Therapy is contracted with Triwest (the VA Community Care Network) and Tricare. Veterans referred through VA Community Care, active-duty service members, and Tricare-covered family members receive the same one-on-one rehab at our Cedar Mill, Tigard, and Milwaukie clinics.

How VA Community Care referrals work

If the Portland VA cannot see you within a reasonable time or distance, you may be eligible to receive physical therapy from a community provider like Creekside through the VA Community Care Network — administered by Triwest in our region. The VA issues a referral and authorization, you call us to schedule, and we bill Triwest directly. Bring your referral paperwork to your first visit.

Tricare patients

Active-duty service members and their families with Tricare can be seen by any of our therapists. Tricare Prime patients typically need a referral from their primary care manager; Tricare Select patients usually do not. Our front desk verifies your specific plan before your visit.

What to bring

Bring your VA referral paperwork or Tricare card, a photo ID, and a list of current medications. New-patient paperwork can be completed ahead of time from our Patient Forms page.
